This position is an opportunity to really MAKE A DIFFERENCE in the lives of the children and families we serve in Bergen County.
We are a growing non-profit social service agency serving youth ages 5 - 21 and their families. The youth served are challenged by serious behavioral, emotional, mental health, substance abuse issues and developmental disabilities.
We are looking for extraordinary people who share our energy and enthusiasm in supporting the mission of Bergen's Promise. The Care Manager position requires care coordination and crisis work, in community, with children and families in Bergen County NJ.
Must be passionate, creative and able to work under supervision and as part of a team.
Must be able to work a flexible schedule to accommodate the needs of the youth and families served.
Requirements -
- Bachelors Degree in a Human Services field - Social Work, Psychology, Sociology, Counseling
- 1 Year of experience working with at-risk youth
The Challenge -
- Ensure the execution of the organization's mission - Healthy Families, Safe at Home - by promoting a culture of exceptional service, strong and coordinated community partnerships and strength through accountability.
- Recruit and retain high-performing Care Managers.
- Professionally develop and provide administrative and clinical case oversight to a team of six Care Managers.
- Accountable for the Care Manager deliverables, which include assessments, development of individualized plans of care, linkages and referrals to services, and monitoring for quality service provision.
Requirements -
- Master's Degree in Social Work, Counseling or related field.
- Current and Valid New Jersey license is a state requirement: LSW, LCSW, LPC, LMFT or LMHC required.
